2008 EPAF Registration Notes

  1. All Members, Extension Faculty, Administrators must pay registration fees. (There are 210 full session scholarships available for county faculty only. Please check the scholarship box on the registration form). To complete registration for the conference you must enter a hotel confirmation number in the required box, even when rooming with someone else (please call them to get the number). If you have checked the scholarship box and you have a 0 balance showing on page 4 of the registration form, please fax that page to 352-392-1908 attention Judy Butterfield. If paying for additional meals, please mail the form along with your check. If paying by PayPal we will receive registration confirmation, so no action is needed.

  2. You must be registered to attend meetings and/or receive awards.

  3. The EPAF Board has instituted a Facilities Fee to encourage everyone to stay at the conference hotel which provides meeting facilities and space for our annual meeting. Before completing your on-line registration form, you must have a Buena Vista Palace guest registration confirmation number to avoid the $100.00 facilities fee. If your county office is within the 50 mile radius of the hotel, or you are paying the daily registration, you will be exempt from this fee. You can contact the hotel at 1-866-397-6516. When registering, please mention you are with the EPAF Conference.

  4. Registration for everyone presenting papers or table top displays must be postmarked by July 14, 2008. This will help with reservations of equipment, tables and rooms.

  5. Payment for registration may be made by printing the online registration form and mailing with an attached check or by using the Pay-Pal electronic payment system with your Mastercard, Visa, or American Express after you have completed the online registration form. Mailing instructions and address are included in Step 6 of the online registration form. You must have completed Steps 4 and 5 before Step 6 appears for payment options. Please complete early enough to allow county or state checks to be processed and mailed. Registration will not be confirmed without a check or online Pay-Pal payment for the full registration amount. An e-mail confirmation will be sent to you when payment is received.

  6. Registration for those presenting but not eating will be $50.00 per day.

  7. Postmarks after July 28, 2008 and on-site registration do not guarantee meals.

  8. Special dietary meal requests must be identified on this registration form.

  9. After the Earlybird registration deadline (July 14, 2008), all refunds will be less a $50.00 fee. Refunds within one week prior to the conference will be reduced by 50%. Once the conference begins, no refunds will be given. Either a letter or e-mail canceling registration must be sent to Judy Butterfield (jrbu@ufl.edu) by August 4th to waive the $50.00 fee.
